ABSTRACT

The giant cell tumor of tendon sheath is a benign histiocytic proliferation of the articular, tendinous and peritendinous synovial tissue that has rarely been reported in the dermatologic literature. Its histopatholgic appearance is characterized by the presence of multinucleated giant cells, histiocyte-like cells, fibroblast-like cells, foamy cells, and hemosiderin. It usually occurs on the fingers, hands and wrists. We describe a case of giant cell tumor of tendon sheath developing on the toe.

ABSTRACT

We report a 33-year-old woman with a 1-year history of typical skin features of dermatomyositis without any evidence of muscle involvement. Her skin eruption of the face (heliotrope erythema) and hands (Gottron's papules) and skin biopsy findings were typical of dermatomyositis. Levels of serum muscle enzymes were within normal ranges and electromyography showed no signs of muscle involvement. During a 1-year follow-up, she had no signs of muscle weakness. Based on these clinical and laboratory findings, our case can be diagnosed as amyopathic dermatomyositis.

ABSTRACT

A 9-year-old male child presented with a 1X1 cm sized cystic nodule and adjacent skin-colored papules on his right shoulder. The lesions were clinically diagnosed as an epidermal cyst and molluscum contagiosum. Histological examination of the cystic nodule showed an epidermal cyst containing molluscum bodies in its wall. We herein report an unusual case of molluscum contagiosum invading a pre-existing epidermal cyst.

ABSTRACT

Apocrine poroma is a benign cutaneous adnexal neoplasm differentiating in the direction of sebaceous and apocrine glands, and follicular germs. The clinical appearance of apocrine poroma is not distinctive, and the histologic finding is similar to that of eccrine poroma, which is typified by proliferation of poroid and luminal cells in continuity with the epidermis. But sebaceous, apocrine or follicular differentiation may also be found in the case of apocrine poroma. We herein report a case of apocrine poroma on the scalp. This case exhibited apocrine and sebaceous differentiation, and connection to an adjacent follicular epithelium.

ABSTRACT

PURPOSE: Recently, Staphylococcus aureus has been reported as the principal microorganism isolated from impetigo, showing variable degrees of antimicrobial resistance. The aim of this work was to study the causative organism of impetigo the their antimicrobial susceptibility developed in impetigo in the past four years. METHODS: We performed bacterial cultures and antimicrobial susceptibility tests in 73 patients with impetigo who visited the Department of Dermatology and Pediatrics at St. Paul's Hospital of the Catholic University of Korea in Seoul. RESULTS: Of 73 patients, microorganisms were identified in 70 patients between May 1995 and August 1999. Staphylococcus aureus was isolated from 63 (90%) patients. Antimicrobial susceptibility test for Staphylococcus aureus revealed low resistance to bactrim (0.0%), rifampicin (0.0%), van- comycin (3.2%), ceftriaxone (4.0%), imipenem (5.4%), ampicillin-clavulanic acid (6.1%), oxacillin (8.1%), cephazolin(9.1%), cephalothin (9.7%) and cefotaxime (10.8%). However, Staphylococcus aureus had high resistance to erythromycin (65.7%) and gentamicin(85.5%). Thirty-six (57.1%) strains of isolated Staphylococcus aureus showed resistance to more than one drug, including two strains (3.1%) resistant to more than 10 drugs. CONCLUSION: Staphylococcus aureus was the most common cause of impetigo developed in the pa- st four years. It was susceptible to most antimicrobial agents except erythromycin and gentamicin, and methicillin resistance was not strong in our results.

ABSTRACT

A 32-year-old woman with a long-standing systemic lupus erythematosus had multiple subcutaneous nodules on her axillae, iliac crests and limbs. Three years ago, these nodules began to appear and slowly became larger. Some of them amassed to form a large, fungating, lobulated mass on her right iliac crest. Roentgenographic and histological examination showed that they were calcium deposits. She was initially treated with aluminum hydroxide administration for nine months, which resulted in moderate decrease in size and softening in consistency, but not complete resolution. Then, the mass on the right iliac crest was excised, with an excellent early result.

ABSTRACT

Classical molluscum contagiosum can be easily diagnosed from clinical examination because of the characteristic cutaneous lesions of umbilicated papules. However, when it occurs as a single growth with atypical presentation, especially on an unusual site, this common disease may give rise to difficulty in diagnosis. Thus, at times histopathological examination is necessary for the proper diagnosis and treatment. We herein describe 5 patients with this solitary molluscum contagiosum, clinically mistaken but histopathologically proven.

ABSTRACT

Two siblings, a 42-year-old man and his 39-year-old sister, have had severe twenty nail dystrophy which began to develop in adulthood. The nails are variably involved and show thickening, ridging, koilonychia, opalescence, and loss of luster. There is no evidence of other skin disorders such as psoriasis, lichen planus, or alopecia areata. No other family members through three generations have had similar nail changes. Our cases are unique in that severe twenty nail dystrophy may concurrently affect two siblings during adulthood.

ABSTRACT

Trichilemmal horn is a unique, uncommon epidermal tumor that exhibits a hard protruding hyperkeratotic plaque. Histologically, it is characterized by striking orthokeratotic hyperkeratosis and verrucous hyperplasia of the epidermis without the formation of a granular cell layer. The acanthotic epidermis shows an epithelial lobule, composed of pale staining keratinocytes, that is contiguous with the overlying epithelium and extended into the dermis. We herein report a case of trichilemmal horn developing on the right temporal area of a 80-year-old woman, which shows typical clinical and histopathological features.

ABSTRACT

Indication for fiberoptic intubation in an awake patient include almost any abnormality that may hinder the expeditious placement of an endotracheal tube during anesthetic induction. An epistaxis is the most frequent complication of nasotracheal intubation. The patient was admitted for open reduction and internal fixation due to severe mandible fracture. We experienced a case of atelectasis due to epistaxis aspiration during awake fiberoptic nasotracheal intubation in the conscious patient regionally anesthetized by both superior laryngeal nerve block and translaryngeal anesthesia, which is treated by saline irrigation, suction, active coughing and chest percussion.
